Improved Prusa MK2 Fan Nozzle

My changes to the original model:

• More space between the nozzles and the hot end to avoid melting
• Walls around the fan output to prevent air leakage
• Nozzle geometry redone to remove thin walls
• Added internal geometry to improve bridging of the top of the duct

Due to the walls around the fan output, the fan must be loosened in order to install this nozzle.
