Decora Keystone Insert like Leviton p/n 41641-W

This is a keystone adapter for a Decora style wall plate.  The corner radii perfectly matches a standard Leviton profile.  A "TOP" label is incorporated to the front and back side for ease of installation.  I recommend printing front side up with top layer ironing, the end result will be very good.  These adapters sell for over $10 apiece which seems expensive.  Printing a few myself was much cheaper and convenient.
